Parents, please fill out this form to add your child to the league list! We'll have a registration card pre-filled out for you on day one! This info will also help us to keep in contact with you throughout the season and during the off-season after league is over.
Please note: You will still need to finalize registration on day one for your bowler's USBC membership and to order their league shirt. Please contact the League Supervisor if you have any questions about registration or league perks.
